Community-based providers and organizations play a vital role in expanding access to tobacco cessation support for Medi-Cal members. That’s why Pear Suite is excited to collaborate with UC Davis Center for Healthcare Policy and Research on this pilot project. Together, we’re supporting Community Health Worker-led outreach and referrals to Kick It California, bridging trusted local relationships with evidence-based care. This project also centers CHW voices in program design, using real-time feedback to shape a more effective and culturally relevant approach. If you're a CHW in California's Central Valley and want to participate, email us at [email protected] or click the link below to sign up! https://bit.ly/CHW-tobacco #CommunityHealth #CHWs #TobaccoCessation

After 10 months of building, recruiting, and partnering with organizations across the country, Pear Suite is proud to officially launch the Pear Cares Provider Network—a growing community of over 1,000 Community Health Workers (#CHWs) from more than 200 community-based organizations, now reaching more than 11 million Medicaid lives across three states. We’ve also partnered with eight Medicaid and D-SNP plans to expand our support across 25 states and counting. One of those plans includes, Health Net—a Medi-Cal managed care organization and company of Centene Corporation—which has partnered with us to recruit and onboard over 300 CHWs to support their 1.6 million members across California. We’re proud to collaborate with leaders like Dorothy Seleski, Senior Vice President for Medi-Cal at Health Net, who helped to champion this important work. This milestone is about more than numbers. It’s about empowering frontline health workers with the tools and support they need to impact communities. Two of our network organizations includes Binational of Central California and Asian Americans for Housing and Environmental Justice, who both believe in our shared goal to make social care more accessible, integrated, and person-centered. We’re celebrating a major step forward in birth equity and maternal health access in the Central Valley! Our valued partners, Ithiopia Mckinney and O Community Doulas, have launched three Birth Equity and Infant Health Mobile Clinics, bringing accessible healthcare to underserved and rural areas. These mobile clinics aim to reduce barriers to maternal care, empower doulas, other birthworkers, and Community Health Workers (#CHWs) with technology to address social determinants of health challenges and improve birth outcomes through trusted, community-led support. Staff will be equipped with the Pear Suite platform, enabling them to provide culturally competent, patient-centered care and get compensated through existing and new funding streams. We’re proud to see this level of collaboration, with support from Dr. Diana Ramos (Office of the California Surgeon General), Dr. Karen Mark (California Department of Health Care Services), and Dorothy Seleski (Health Net), alongside our Head of Community Health, May B Ly. Thank you to all who made this possible, and to the incredible doulas and Community Health Workers who are already on the front lines improving lives in these communities.

Community Health Workers are the bridge between communities and healthcare, but too often, they’re forced to work within systems that weren’t built for them. Outdated processes, limited resources, and administrative hurdles slow them down, taking time away from the people they serve. At Pear Suite, we believe technology should empower, not hinder, #CHWs. With the right tools, they can spend less time navigating barriers and more time making an impact where it matters most. In Healthcare IT Today, our CEO and Co-founder Colby R. Takeda discusses how tech can eliminate obstacles and amplify the impact that CHWs have on their communities. Read the article: https://lnkd.in/gr9BvvPM #HealthEquity #CommunityHealth

Community Health Workers (#CHWs) bring more than just knowledge—they bring lived experience that allows them to connect with communities in powerful ways. This perspective impacts healthcare in three key ways: 1?? Building Trust – CHWs can relate to patients through shared experiences, creating stronger connections and improving engagement. 2?? Improving the Experience – CHWs' lived experience helps them better understand the needs of the community, allowing for more tailored and effective support. 3?? Driving Policy Change – Lived experience drives CHWs to advocate for underserved populations, helping to create lasting change. Lived experience is priceless. It’s the foundation that allows CHWs to truly make a difference in healthcare.

Community Health Workers (#CHWs) are the backbone of healthcare access for millions. They build trust, bridge gaps, and drive meaningful impact where it matters most. Yet, they remain underfunded, under-supported, and under-recognized. In his latest MedCity News op-ed, CEO and Co-founder, Colby R. Takeda, highlights the urgent need to provide greater support and recognition for this vital workforce. Read more below?? #HealthEquity
? In my latest op-ed published in MedCity News, I highlight Community Health Workers (CHWs) as the unsung healthcare heroes—bridging gaps, building trust, and driving real impact in communities. Yet, despite their critical role, they remain underfunded, under-supported, and under-recognized. I also explore the immense potential of #CHWs and emphasize the urgent need for bold leadership to fully support and elevate this vital workforce. If we are serious about transforming healthcare, we must begin by empowering those who are already making the greatest impact where it matters most. ??: https://lnkd.in/gmc6mUu5 I’d also love to hear your thoughts—how can we better support CHWs and drive meaningful innovation in healthcare? #HealthcareLeadership #HealthEquity #MedCityNews #communityhealthworkers Pear Suite #pearsuite

Access to technology is critical for ensuring older adults have the resources they need to stay connected to their health, communities, and essential services. That’s why the #Kūpuna Collective, co-coordinated by the Hawai‘i Public Health Institute and the University of Hawaii at Manoa Center on Aging, is leading efforts to address digital inclusion across Hawai?i. The Benton Institute for Broadband & Society highlights how the Collective has united 470+ individuals and 274 organizations to support Hawai?i’s aging population. At Pear Suite, we’ve been proud to contribute—led by David Mamae—by training multilingual digital navigators across the Hawaii State Public Library System, helping kūpuna improve their digital literacy and connect to critical services.
