Dates: Tuesday, March 18-May 27, meeting every other week Time: 2-3:00 p.m. EST This virtual workshop series provides healthcare professionals of all disciplines the space to practice Patient Priorities Care through discussion and case-based coaching. Not only is this workshop series free for all health professionals to attend, but participants can also earn six continuing education credits. VIRTUAL CONFERENCE | Tune in for our National PACE? Association 2025 Virtual Winter Conference session, "Elevating Person-Centered Care Planning with the Final Rule." ?? Friday, February 14, 01:00 - 02:15 PM EST ?? Session Details: PACE Organizations are aligning with the Final Rule and looking for ways to enhance their patient-centered care planning significantly. This presentation will focus on integrating patient priorities within care plans, highlighting how our approach addresses the challenges posed by the Final Rule and demonstrates PACE organizations' commitment to maintaining meaningful, participant-focused care. In this talk, the speakers will begin with a shared theoretical framework, emphasizing change management, a flexible approach to Patient Priorities Care, age-friendly practices, and a shared commitment to learning and adaptation. Our speakers will showcase four unique PACE Organizations, three distinct EMRs, and their strategies for improving care plan processes in consideration of the Final Rule using Patient Priorities Care. Each team will present a PACE case study, demonstrating how the plan of care was (1) co-created with participants and their caregivers, (2) built on personalized goals and care preferences, (3) addressed 13 key areas of care and chronic conditions, and (4) functioned as living documents. We will highlight cross-disciplinary interactions within the plan of care. These real-world examples will show the challenges and successes of transitioning to a more holistic, participant-driven approach to care planning. By sharing their experiences, challenges, and successes, they aim to foster a collaborative learning environment among PACE providers. Together, we can ensure that our care plans genuinely reflect the needs and preferences of our participants, ultimately improving outcomes and sustaining the integrity of the PACE model. ?? If you celebrate Thanksgiving and are visiting older family members, consider having a conversation about their health priorities. In honor of National Family Caregivers Month, we spoke with Barry Wu about his experience long-distance caregiving for his mother and how identifying her health priorities has changed her care. "As a son, I felt like I knew what my mother wanted," says Barry. "But when we went through the questions together, I learned things about her I didn’t know." Understanding what matters most to your older loved ones can help change their care so they can spend their time doing what they value most: Maybe it's living as long as possible, volunteering at church, living independently, or eating delicious food. "As a caregiver, you need to hear what their values and health priorities are, not your own priorities. You want to hear what’s most important to them and do things that respect their dignity," says Barry. ??
