Transformative. Community Based. Outcomes Focused. The Pathways Community HUB Institute (PCHI) was founded on a simple principle: achieve health equity. PCHI exists to assist communities in creating a sustainable community-based care coordination network that supports their under-resourced residents and connect them to services, track outcomes, and contract with payers that directly tie payment to those outcomes. Through the Pathways Community HUB Institute? Model (PCHI? Model), communities are provided with the framework and support to build a robust, complete network of care. The PCHISM Model is focused on the whole person and working in in partnership across multiple community stakeholders to align efforts and resources to reach the common goal of good health for all. PCHI knows first-hand the impact that a community-designed care coordination network can have on individuals and their families. Today, the PCHI? Model is being implemented in 40 communities in 17 states and counting.

Congratulations to the 1889 Jefferson Center for Population Health on receiving the Highmark Bright Blue Futures Award from the Highmark Foundation for the impactful work being done through its Community Care HUB (CCH). Launched in 2020, the CCH utilizes the PCHI Model to coordinate care for individuals with complex needs in Cambria and Somerset counties in Pennsylvania. Since its inception, the CCH has received over 2,200 referrals, made more than 10,000 contacts, and enrolled over 1,000 participants. The HUB’s community health workers have opened over 5,000 pathways to address health and social needs, including transportation, education, employment, and stable housing. This proactive approach is significantly improving health outcomes and advancing health equity. In 2023, the Williamson County Pathways Community HUB (United Way for Greater Austin), officially launched in the Greater Austin, Texas area. Since then, it has made strides in supporting pregnant and postpartum mothers and their infants. Through partnerships with trusted organizations like STARRY and Impact Counseling, the Wilco PCH has been actively providing care coordination, health education, and advocacy to those at high risk for poor health outcomes. The team is currently accepting referrals from various local health care providers, focusing on areas with the highest need. Congratulations to the entire Williamson County team for their dedication and impactful work in improving health equity for families in the community! After an incredible 38-year career, including three years as President & CEO of PCHI, Jan Ruma will be retiring on October 20, 2024. During her tenure, Jan has been instrumental in expanding our network to nearly half of all U.S. states and 50 communities, reaching over 40,000 people annually with whole-person, community-based care coordination.? Community health workers, who share similar health and life experiences with those they serve, play a crucial role in assessing and caring for people with complex needs. However, many community-based organizations employing CHWs often lack the infrastructure needed to establish contracts with health care entities. This is where the PCHI Model steps in, helping communities coordinate the administration and infrastructure of a network of CBOs, enabling partnerships with health care organizations. CHWs are vital to this network, supporting underserved residents.
