Concerned with length loss and subsequent metatarsal head elevation when doing a 1st TMT fusion? Consider the Bun-Yo-Matic? Lapidus Clamp from Paragon 28. The Bun-Yo-Matic? Lapidus Clamp System is equipped with cutting guides designed to remove minimal bone from the base of the first metatarsal and the medial cuneiform to prepare the joint for fusion. The guides help establish parallel cut surfaces to improve apposition and arthrodesis following correction. Surgeons make a guided flat cut on the metatarsal base, translate and derotate the first metatarsal, and then make a corresponding parallel cut on the base of the cuneiform minimizing length loss when compared to many of the competitive systems on the market. The design of the clamp and cut guides allows for guided corrections in all three planes while maintaining fluoroscopic visualization of the first metatarsal, sesamoids, and first TMT joint.
