With over 16 years of dedicated service in the ever-challenging field of Social Services, Brianna has supported countless patients through discharge planning, focusing on those who are sick, afflicted, and underserved, as well as facilitating transitions to many lower levels of care. Social Services plays an essential role in our team, bridging the gap from acute care to post-acute care and ultimately to home. Brianna’s contributions have been instrumental in the growth and success of PACS. It was her first time to the main office. It was humbling to witness her surprise when she walked through the doors and met the many faces behind PACS, she left truly inspired by our growth. ????
