LASER Class 101 Yesterday I responded to a great post from Bryan Saltzman ?and Jan S. on Orthobullets on an interesting ACL case. I wanted to take this opportunity to share, based on the science, the clinical benefits of using laser after surgery. Our acute inflammatory healing reaction has evolved over hundreds of thousands of years. Inflammation after acute injury starts with a “shock and awe” response. After being bitten by a saber-toothed tiger our evolutionary success required us to stop the bleeding and seal the wound asap. This acute painful aggressive inflammatory response is dialed in to our DNA to recede within 72 hours. Surgery is acute iatrogenic traumatic inflammation; it is a modern-day tiger bite. The biggest question is what happens after the first 72 hours. There are two inflammatory pathways that can occur: chronic inflammation or acute healing. The chronic pathway creates dense painful avascular scar tissue at the incision and below. This translates into painful scars, reduced range of motion, prolonged pain and swelling. There is a breakdown in the cellular messaging to transition to acute healing. As surgeons our goal is to transition patients to the acute healing phase as soon as possible. Laser/Photobiomodulation facilitates that transition. The basic science of PBMT cellular reactions is well established. By shining the correct wavelength of light into our cells the following happens: 1.???????Increased Mitochondrial production of ATP 2.???????RNA Transcription of Healing Anti-Inflammatory Cytokines 3.???????Fibroblastic Ingrowth 4.???????Vascular Formation and Increased Blood Flow (Galactin Protein) 5.???????Neurogenic Healing (HNRNP K Protein) 6.???????Myoglobin Production: Muscle Healing 7.???????Edema Reduction We are a successful species that thrives on the surface of a planet that exists in visible light. It should not come as a surprise that in our deepest genetic code we are sensitive to light. I have included the results of our National OrthoLazer postoperative patient reported outcomes survey. 96% of our patients recognize the benefit of postop laser.
