Revelation in speaking with a patient last night, the Lilly cares program is as close to ?? % vertical integration as possible, meaning Lilly partnered with a national mailing pharmacy which is a “ships to you” disservice & has all but abandoned local care at your local pharmacy who is your local goto for whatever ails you or your local source for pharmaceutical care or local wellness (get the point ???) Can someone please explain to me why with all of the “efficiency” brought in the vertically integrated program, why the cost is $550 to the patient? The API {active ingredient}, the pen, the box, the shipping, all in is under $50 cost. Where is the $500 profit going? How much tax is the government collecting? Why is the WAC {wholesale acquisition cost} so high? $1059.87 ?? In adjacent but not so breaking ??? news ??? Why would ????Weight Watchers abandon the brand name drugs & team up with a 503B entity to produce GLP1’s? There is no excess ??????tax… I mean excise tax in the 503B space folks! Only income tax for the owners! Also, did you know that the wholesale industry can’t touch the invoicing process of 503B? That’s right! No perversion of price by wholesalers! But beware! GPO functions like Premier are trying to get their greedy hands in the mix! Keep pushing for transparency! 503A compounding & 503B let’s call it botuque manufacturing/compounding is the correct path until manufacturers can break the enslavement chains of federal tax & wholesaler taskmasters!

Pharmacists, we are under assault from “AI” at an entirely new level than our former “webMD” or “Google” adversaries! A great lecture in the patient care experience from Creighton University college of #pharmacy was knowing when to say when: “I will have to research & verify & get back to you with an answer,” is far better than giving up some half baked notion to save face. AI does just this. With some facets, AI will just fill in data with notions or complete rhetoric. This can be our edge. This is why you want a good #pharmacist in your life. We had appropriate processes drilled into us. Patients??Warning signs of a poor pharmaceutical care experience: 1. Rare encounters with a pharmacist- simply not available is no bueno 2. Nameless- does the pharmacist know your name? “Cheers… Norm” (dating myself here) they should “know your name” 3. Followup- impossible to know & cover all aspects of your plan in one sitting, followup should occur, if not, run 4. Precision- you are not a number, we have guidelines, but pharmacist can facilitate personalized plans to deploy with YOUR team.
