What does it take to make collaborative care work? The guiding principles exist – a patient-centered care team, evidence-based care planning, reimbursement based on quality of care and outcomes, etc. – but making those principles work in reality can be challenging. And with over 80 models of "integrated care" in the market, finding the right approach can be overwhelming. That is why our team recently took a closer look at three different collaborative care models that are up and running at Kaiser Permanente, which serves 12.6 million members across 775 hospitals. These models are driving positive results for both outcomes and costs. Take a look at our executive briefing coverage of the models and access the original presentation, “Unpacking The Executive’s Toolbox For Building Collaborative Care Models,” led by Dr. Stuart Buttlaire, Regional Director Of Behavioral Health & Addiction Medicine at Kaiser Permanente: https://lnkd.in/eQuZGbgS #CollaborativeCare #Healthcare #HealthcareInnovation #MentalHealth #BehavioralHealth #WholePersonCare #PatientCenteredCare #CollaborativeMedicine #PatientCentric
