OneCity Health is a subsidiary of NYC Health + Hospitals focused on population health, care management, and implementation of the State��s Delivery System Reform Incentive Payment (DSRIP) program. Established in 2014 under the auspices of the DSRIP program, OneCity Health envisions the establishment of a welcoming, accessible, and integrated health delivery system that encourages, supports, strengthens, and protects a state of wellness and healthy living for all. Our transformation efforts include: ? Implementing DSRIP initiatives together with NYC Health + Hospitals and over 200 partner organizations representing primary care providers, community-based organizations, behavioral health providers, and SUNY Downstate Medical Center - University Hospital of Brooklyn ? Improving access to primary care ? Enhancing care management and care coordination for New Yorkers with complex health and social needs ? Improved treatment and support for persons with mental health or substance abuse challenges ? Integration of health and social services ? Delivering culturally competent care ? Increasing outreach to patients who have had difficulty connecting with care NYC Health + Hospitals is the largest public health care system in the nation serving more than a million New Yorkers annually in more than 70 patient care locations across the city��s five boroughs.
