Our History and Mission Formerly Health Care for the Homeless, Outreach Community Health Centers (OCHC) has, at the heart of its mission, a drive to provide the highest quality health care and support services to the most vulnerable people in the Greater Milwaukee Area: the homeless, those experiencing extreme poverty and the uninsured. Incorporated in 1980, Outreach Community Health Centers began direct service delivery in 1984 as a demonstration project funded by the Robert Wood Johnson Foundation and the Pew Memorial Trust. During the past 35 years, OCHC programs have grown to provide a strong continuum of services for the homeless as well as a critical complement of support services for the poor and uninsured. Today, OCHC delivers services to those in need at six different locations, while our Street Outreach team seeks and finds homeless people sheltered in abandoned buildings and parks, as well as those seeking service at area shelters, food pantries and meal sites. OCHC serves the most disadvantaged people in Milwaukee County. Residents of the 5 poorest zip codes in Milwaukee County make up 40% of OCHC’s service population, and the zip code of our primary service site, 53206, has a median household income that is 48.7% lower than that of the county as a whole.

The 2025 Open Enrollment Period (OEP) begins November 1, 2024, and ends January 15, 2025. More than 20 million children in the U.S. still lack sufficient access to essential health care. The MCW Family Medicine Residency Program at Outreach Community Health Centers has committed over $2.4 million to expansion and renovation of its 210 facility to accommodate the number of residents and anticipated increase in services. The collaboration between MCW and OCHC will also enhance the mission of OCHC through future street medicine and other projects and research positions. The mission of Outreach Community Health Center is to promote health and wellness to all individuals that we serve.?This collaborative plan strengthens this mission parameter in that it allows OCHC to build upon the premise by reaching all individuals to include the homeless, and at-risk; as well as the underprivileged and underserved. We must invest in primary care at a level that reflects its immense value. Primary care is the only part of the healthcare delivery system where an increased supply is tied to better population health and equitable outcomes. Yet, as the Milbank Memorial Fund has documented, the United States continues to underinvest in primary care. Currently, national spending on primary care is only 5-7 cents of each healthcare dollar.
