Nutrition Unlimited provides evidenced based medical nutrition therapy and diabetes education that focuses on awareness, prevention and intervention across a broad spectrum of medical diagnoses. Through partnerships with various organizations we deliver our services at medical offices, corporate worksites, virtually, and in our center city office. Our mission is to help medical/therapy practices and employer to improve health outcomes and reduce healthcare costs. Medical providers depend on us as their nutrition referral source because we specialize in customized programming that targets health improvement and chronic illness prevention/management. We contribute to The Comprehensive Primary Care (CPC) initiative and it’s five core functions: 1) Risk-stratified Care Management; 2) Access and Continuity; 3) Planned Care for Chronic Conditions and Preventive Care; 4) Patient and Caregiver Engagement; and 5) Coordination of Care across the Medical Neighborhood. In support of the CPC model, Nutrition Unlimited corresponds with Primary Care Providers keeping them aware of our nutritional recommendations to help enhance patient experience. Our team of skilled Registered Dietitian Nutritionists and Certified Diabetes Educators participate with most commercial health plans and offer dietary strategies, conveniently provided at no or low out of pocket cost. We focus on increased positive clinical outcomes, lower healthcare costs, reduced medication use, and hospital admissions. With nutritional intervention we have been proactive in providing a robust offering of solutions that help increase clinical quality and utilization measures that drive total cost of care.
