A physician or healthcare provier would refer the patient to Nursing Clinic, PLLC to receive Home Infusion Therapy. The referral is coordinated with the Polaris Specialty Pharmacy. Once the pharmacy dispenses the medication and the supplies directly to the patient’s home, our nurses are sent to provide the infusion therapy in the comfort of the patient’s home.
