It’s nearly spring and there's no better time to talk about bacterial vaginosis (BV) – the infection that suddenly shifts the natural balance of things and shakes things up! As we know, persistent or recurrent BV is common and there’s limited data regarding management strategies for women, especially given how complex the polymicrobial biofilm is on vaginal epithelial cells. This week the New England Journal of Medicine released the results from a new clinical trial about BV and the use of male-partner treatment to prevent recurrence in their female partners. The study included monogamous couples in which the female was treated for BV with standard first-line antimicrobial agents, and their male partner was treated with topical 2% clindamycin cream (applied to the penis) and oral metronidazole 400mg (both twice daily for 7 days). When examining recurrence rates, the researchers found that recurrence only occurred 35% of the time in the partner-treated group, compared to 63% in the control group (where only the female was treated). Previous research into the prevention of BV recurrence has looked at the treatment of male partners with oral antimicrobials alone. Most recently, in 2020, a double-blind study treated men with the same course of oral metronidazole as their female counterparts (500mg twice daily for 7 days) and found no statistically significant difference between the two treatment arms (treatment vs placebo). This study concluded what was found in prior studies showing no benefit to treating the male partner. This newly released study is interesting as it suggests that BV may be harbored or transmitted by a woman’s male partner, and that clearance of the bacteria could require a combination of oral and topical antibiotics. It’s worth repeating; possibly with controls for other possible contributing factors (i.e., intra-uterine device use, presence of circumcision, smoking status, and vaginal douching or lubricant use). The female biofilm conundrum is tricky, and perhaps it’s now evident that oral treatment alone doesn’t clear the bacteria from all male sources (penile skin, urethra, semen and urine).
