Timothy*, a retired contractor, found himself in an unexpected crisis when his landlord sold the property he was living in, leaving him homeless and without stable resources. With no primary care provider or dentist, and struggling to navigate his new reality, he was referred to our Community Health Team by a local partner. Our team quickly stepped in, helping Timothy establish visits for primary care and dental care, connecting him with a housing partner to secure a place to live, and with Valley Health Connections to get enrolled in Medicare. In addition to medical support, we connected him with utility assistance, diabetic wound care, and access to transportation, giving him the resources to regain his health and dignity.
