We followed the journey of an elderly outpatient with high blood pressure and uncovered key gaps: ?? Key Gaps Identified: ? Incomplete documentation in the HMIS, leaving out critical medical history ? Misaligned communication between departments, leading to delays ? Lack of structured follow-up, affecting continuity of care ?? Here’s What We Did: ? Introduced streamlined documentation protocols for high-risk patients ?
