Having choices is great—until it’s overwhelming. For those with employer-provided health insurance, choosing between a high-deductible or low-deductible plan can feel stressful enough. Now imagine facing over 100 options. They hand you a bottle of wine and a compass: “Good luck.” This is what employees can experience with ICHRAs (Individual Coverage Health Reimbursement Arrangements). With an ICHRA, employees pick their own coverage through the marketplace, and employers reimburse them. ICHRAs have their positives—especially for small employers that previously couldn’t offer benefits or faced unsustainable cost increases. They provide a viable option with predictable subsidies. In some cases, something is better than nothing. But ICHRAs come with challenges that go beyond choice overload: → Narrow networks — Many hospitals nearby may be out of network, forcing employees to travel far for care. → Higher costs — Due to adverse selection, exchange plans are often more expensive with substantial out-of-pocket costs. Even “premium” plans may leave employees vulnerable. → No cost control — ICHRAs shift the burden to employees to select and manage their plans wisely, but they don’t address rising healthcare costs.
