The Importance of Home Tonometry in Glaucoma Care Intraocular pressure (IOP) dynamics are critical in managing glaucoma, yet traditional in-office measurements provide only a limited snapshot. Understanding and addressing IOP fluctuations, generally occurring in the early morning outside office hours, is essential for effective glaucoma care. Glaucoma specialists Dr. Ruth Williams, Dr. Ike Ahmed, Dr. Michael Chaglasian, and Dr. Leon Herndon discuss the advancements and benefits of home IOP monitoring. Dr. Ruth Williams, Wheaton Eye Clinic, emphasizes the dynamic nature of IOP, comparing it to heart rate and blood pressure. She highlights the limitations of infrequent office measurements and the importance of understanding IOP variations throughout the day and night. She underscores the significance of home tonometry for patients who appear well-controlled but continue to progress. Detailed data on pressure spikes can support changes in management, timely surgical interventions, and improved outcomes. Dr. Ike Ahmed, John A. Moran Eye Center, explains that IOP is our only modifiable risk factor and stresses the need to measure IOP beyond the office. Harmful IOP peaks often occur outside office hours, potentially leading to significant glaucoma damage. He elaborates on the practical implementation of home tonometry, which can enable us to be proactive before damage occurs. By encouraging patients to use the device consistently, they can obtain a detailed pattern of their IOP, which informs customized treatment plans. Dr. Michael Chaglasian, Illinois College of Optometry, discusses the value of home tonometers in capturing comprehensive eye pressure readings. He notes that identifying peak pressures and understanding IOP fluctuations are crucial for assessing the risk of vision damage from glaucoma. Dr. Leon Herndon, Duke Eye Center, shares his positive experience with MyEyes and the iCare HOME2, highlighting its seamless integration into patient care and the valuable feedback received from patients. Experts agree that home tonometry is transforming glaucoma management, providing better real-world pressure guidance and reducing patients' uncertainties. More and more glaucoma specialists are now using home tonometry before and after surgery, enabling optimized post-operative management to ensure surgical success. This innovative approach enables personalized and more effective treatments, ultimately enhancing patient involvement, care, and outcomes. Recent research expands our understanding of intraocular pressure (IOP) as glaucoma's primary modifiable risk factor. The publication "The Relationship Between Intraocular Pressure (IOP) and Glaucoma: An Evolving Concept" synthesizes findings on IOP dynamics, technological advancements in tonometry, and implications for glaucoma management ?? While elevated IOP has long been linked to optic nerve damage and glaucomatous progression, IOP-related risk is now recognized as multifaceted and influenced by the structural and biomechanical characteristics of the optic nerve head. 1?? IOP & Glaucoma Pathogenesis Elevated IOP causes progressive damage to retinal ganglion cells (RGCs), especially at the optic nerve head, where biomechanical stress impedes axoplasmic flow and induces cell apoptosis. IOP susceptibility varies, with some developing glaucoma at normative IOP levels, highlighting individualized risk. 2?? IOP Fluctuations & Disease Progression Clinical studies indicate that IOP fluctuations, particularly peak IOPs outside office hours, are independent risk factors for glaucoma progression. Traditional in-office tonometry provides a limited assessment, potentially missing critical fluctuations. 3?? Advancements in Tonometry Home tonometry devices offer a more comprehensive temporal IOP profile. This technology better aligns with chronic care models, like continuous blood pressure monitoring and glucose management. 4?? Ambulatory IOP Monitoring for Improved Management Ambulatory IOP monitoring captures real-time data on fluctuations, facilitating personalized treatment. Home-monitoring devices can enable proactive adjustments in therapy, potentially improving outcomes. 5?? Clinical Trial Insights on IOP Reduction Landmark trials, including OHTS and CIGTS, show that IOP reduction correlates with slower disease progression, underscoring the importance of sustained, individualized IOP targets to preserve visual function. 6?? Future Directions in Glaucoma Care Continuous IOP monitoring has the potential to transform glaucoma management. Prospective studies are needed to verify whether tonometry-based care models reduce vision loss. Advanced IOP monitoring offers nuanced insights into how IOP influences glaucoma pathophysiology. Thank you to the authors for their continued research and mission to drive glaucoma management forward:?Barbara Wirostko MD, FARVO, Thomas V Johnson, Elyse McGlumphy, MD,?Lama Al-Aswad, MD, MPH, Craig Chaya, Shan Lin, M.D., David Musch, Ian Pitha-Rowe, Alan Robin & Sanjay Asrani.

Understanding the "Silent Thief of Sight," Glaucoma ?? Glaucoma is a leading cause of blindness and often shows no early symptoms. ?? Increased intraocular pressure (IOP) is a critical factor that damages the optic nerve. ?? Regular eye exams are essential for early detection and prevention of irreversible vision loss. ?? People over 60, those with a family history, and certain ethnic groups are at higher risk. ?? Treatment options include medication, laser therapy, or surgery to reduce eye pressure. Did you know 1?? Researchers at The Johns Hopkins University (Johnson Laboratory) have found that IOP spikes often occur between 4 a.m. and 6 a.m. when patients are not typically seen in a clinical setting. 2?? Any patient with glaucoma who is on medications or has had changes in therapies would benefit from knowing their IOP at various times of the day. 3?? Like other chronic diseases, such as diabetes and high blood pressure, better and tighter control leads to better outcomes. 4?? Frequent monitoring at different times of the day provides a more comprehensive view of IOP variations, which can lead to more effective treatment plans. 5?? MyEyes can help answer questions about monitoring your IOP at home using the iCare HOME2 today. Visit our website or call us at 1-888-959-5563 (Monday through Friday, 9 a.m. to 7 p.m. EST).
