Recent research expands our understanding of intraocular pressure (IOP) as glaucoma's primary modifiable risk factor. The publication "The Relationship Between Intraocular Pressure (IOP) and Glaucoma: An Evolving Concept" synthesizes findings on IOP dynamics, technological advancements in tonometry, and implications for glaucoma management ?? While elevated IOP has long been linked to optic nerve damage and glaucomatous progression, IOP-related risk is now recognized as multifaceted and influenced by the structural and biomechanical characteristics of the optic nerve head. 1?? IOP & Glaucoma Pathogenesis Elevated IOP causes progressive damage to retinal ganglion cells (RGCs), especially at the optic nerve head, where biomechanical stress impedes axoplasmic flow and induces cell apoptosis. IOP susceptibility varies, with some developing glaucoma at normative IOP levels, highlighting individualized risk. 2?? IOP Fluctuations & Disease Progression Clinical studies indicate that IOP fluctuations, particularly peak IOPs outside office hours, are independent risk factors for glaucoma progression. Traditional in-office tonometry provides a limited assessment, potentially missing critical fluctuations. 3?? Advancements in Tonometry Home tonometry devices offer a more comprehensive temporal IOP profile. This technology better aligns with chronic care models, like continuous blood pressure monitoring and glucose management. 4?? Ambulatory IOP Monitoring for Improved Management Ambulatory IOP monitoring captures real-time data on fluctuations, facilitating personalized treatment. Home-monitoring devices can enable proactive adjustments in therapy, potentially improving outcomes. 5??
