Today I got to meet my (apparently) long lost philosophical twin sister, Brooke Meehan. Like me, Brooke has designed her work to invite grownups to talk about how they want to live until they die. She has carved out spaces to improve on traditional advance care planning - which focuses largely on irreversible conditions and inability to communicate - with personal, guided conversations about who someone is, what they value, what makes life precious, and how they want to approach changes to their abilities. Both of us used the heartbreaking observations from our nursing work to inform our passion and creativity in making something better. No lie, at one point I had been talking so long I got short of breath. There were no breaks in the conversation. I can't wait to partner with her on better ACP. #advancecareplanning
