We are currently seeking a full time RN care manager to manage and coordinate the patients plan of care for our Emergency Department. This consists of their admission, active phase of care and addresses discharge needs and concerns to ensure appropriate, cost- effective, quality care. ? You will: ·????????Actively promote primary and secondary prevention of cardiovascular disease in patient population and community by education and example. ·????????Develop, implement, and evaluate exercise programs for patients. ·????????Assess cardiovascular and metabolic effects of exercise for cardiac and musculoskeletal ·????????rehabilitation. ·????????Create educational materials and sessions for patients. Mount Sinai South Nassau’s new, expanded #FennessyFamily #EmergencyDepartment opens tomorrow, Wednesday, March 5 at 9am! The entrance for drive-up or walk-in patients has moved from Washington Avenue to the east side of Oceanside Road south of Oswald Court and north of Nassau Parkway, while the entrance for ambulances remains on Nassau Parkway. The $50 million ED renovation doubles its previous footprint and capacity and is the cornerstone of the new four-story, 100,000-square-foot Feil Family Pavilion that will include nine new operating rooms and forty new critical care suites set to open later this year. The ED will be able to see approximately 75,000 patients annually and features centralized nursing stations for direct oversight of patient rooms, bedside #triage, a state-of-the art #traumaunit with an adjoining #radiology bay, and separate areas for pediatric and behavioral health patients, all designed to reduce wait times and improve the patient experience. #LongIsland #EmegencyRoom #Oceanside

The Patient Experience Coordinator is in a unique position to observe and influence processes and behaviors that result in patients/families feeling safe, well cared for and cared about. You will engage the staff in experiential learning and provide direct observation with appreciative coaching on recommended best practice techniques. The objective of this role is to promote positive employee engagement in relationship-centered care through active listening, clear closed loop communication and teamwork, addressing what matters most to each individual patient/family. The relationship of patient safety, high quality care and a positive experience will be understood by the staff. Success will be measured using quantitative data (HCAHPS), qualitative feedback (Voice of the Patient) in all forms, and Nurse Manger/staff satisfaction. We require: ·????????Bachelor’s degree in healthcare, organizational development, counseling, hospitality, or related field required. Master’s degree preferred. ·????????Minimum 2 years of experience in a related field, with demonstrated experience coaching and supporting others to achieve goals. Experience in healthcare environments preferred. ·????????Excellent interpersonal skills ·????????Excellent written and verbal communication skills Salary $ 33.21/hour.
