The Cohen Laboratory at the Icahn School of Medicine recently published their work entitled “Reparative immunological consequences of stem cell transplantation as a cellular therapy for refractory Crohn’s disease” in the journal GUT: https://bit.ly/41Ip7O9 . Patients with Crohn’s disease that failed to respond to traditional medications or surgery underwent an autologous stem cell transplant with over 90% of patients demonstrating at least partial healing of their intestine. Patients provided samples throughout the stem cell transplant process to help investigators understand the therapeutic mechanisms of stem cell transplantation and how hematopoietic stem cells can be used to restore a healthy immune system. The results of these studies suggested stem cell transplant restored healthy intestinal macrophages needed to support intestinal healing which may be defective in medically refractory Crohn’s disease. Interestingly, this study suggests Crohn’s disease may impair hematopoietic stem cells suggesting an entirely new pathophysiology relevant to the development of Crohn’s disease therapies and diagnostics. This work was accomplished through a multidisciplinary collaboration at Mount Sinai including the Feinstein Inflammatory Bowel Disease Center and the Tisch Cancer Institute. It was wonderful to present a plenary talk about the initial outcomes from the International Crohn's Disease Stem Cell Transplant Consortium at ECCO European Crohn' and Colitis Organisation meeting in Berlin. We will continue our work to understand the long-term clinical outcomes of stem cell transplant and to create standardized clinical protocols for patients with Crohn's disease. This work could never have been done without an incredible collaboration between physicians and scientists at Mount Sinai Division of Gastroenterology (Jean-Frederic Colombel, Daniela Guisado), Hospital Clínic de Barcelona (Azucena Salas, Elena Ricart), Queen Mary University of London (James Lindsay), and UMC Utrecht (Bas Oldenburg, Herma Fidder).
