Mobile Health Providers, APC

Founded in 2021 by Dr. Wyzscx Patacxil, Mobile Health Providers is a mobile primary care practice delivering health care to five counties across Southern California. We work closely with many hospice and home health companies to ensure continuity of care between the hospital, SNF/assisted living/board & care, and private home settings. Our vision is to fill the gap in care that often occurs in homebound patients and decrease readmissions back to the hospital and SNF by proactively managing chronic diseases before they become acute illnesses.
