The NC Department of Health and Human Services is investing $5 MILLION to expand the Collaborative Care Model (CoCM) in primary care offices across the state. Up to $50,000 per site is available for providers, especially in rural and underserved areas, to help build capacity for and implement the Collaborative Care Model (CoCM). CoCM is proven to increase access to behavioral healthcare and improve patient outcomes, all while generating additional revenue.
