Implementing Collaborative Care (CoCM): The Right Team, Technology, and Strategy for Lasting Success Implementing CoCM takes time, energy, and the right team to succeed. Beyond bringing in the necessary staff (like Care Managers and Consulting Psychiatrists), it requires thoughtful implementation and leveraging the right technology. Sure, it’s possible to use a spreadsheet to implement CoCM—but we don’t recommend it. Why? Because you need a system that integrates directly with your EHR and provides the tools a Care Manager needs to be effective. A Care Manager needs streamlined workflows, easy access to patient data, and the ability to collaborate across the care team seamlessly. “She felt like she was being treated ‘like a worker in a factory.’” This powerful quote from Dr. Amy Fogelman in an article by the Boston Globe Media embodies the stark reality physicians face today: shifting from helping patients heal to constantly navigating the maze of insurance companies and billing systems. The article details how physicians are experiencing burnout not just from patient care, but from the increasing pressure of dealing with the complexities of insurance and billing, which ultimately detracts from their passion for medicine. Dr. Fogelman, trained to treat patients, was forced to learn the world of insurance and faced a plethora of billing related issues during her career, most out of her control. She spent countless hours helping patients navigate their insurance, often taking the blame for a system she didn’t design. This personal experience is part of a larger, concerning trend in healthcare. A study published in JAMA, Journal of the American Medical Association, in 2023 outlined that one-third of physicians surveyed reported a “moderate or greater intention to leave” the profession. According to a report by Definitive Healthcare, 6% of our entire physician population in the US, 70,000 physicians, left the workforce from 2021 through 2022. A recent study by West Health and Gallup found that 70% of Americans want their primary care provider to ask them about both their physical health AND their mental health. The National Institute of Mental Health (NIMH) estimated that in 2022, 23.1% of the United States adult population, 59.3 million Americans, were living with a mental illness. So why did only HALF of these individuals receive mental healthcare? In no particular order: ?? Access ?? Stigma ?? Affordability We’re working to solve the access problem through collaborative care and contrary to popular belief, it’s more affordable for patients than traditional CBT or psychotherapy. Many payers are even waving the copays for patients enrolled in collaborative care. While there's still a long way to go in breaking down the stigma surrounding mental health, having open and honest conversations with loved ones and treatment providers is a crucial first step.
