Minnesota Network of Hospice & Palliative Care (MNHPC) is the state's leading palliative and hospice care network. For more than 30 years, our work has helped shape how palliative and hospice care services are offered and delivered. We bring providers, business partners, individuals and donors together to increase knowledge, access services, and strengthen advocacy for people living with a serious illness or experiencing the end of life. Families and individuals rely on MNHPC to learn about their options and guide them to proven programs and trusted partners. We work in collaboration with regional organizations and national leaders to provide a state-wide perspective on advance care planning, palliative care and hospice care. In addition, we offer providers opportunities to further their professional knowledge.

It is with deep sadness that we share the passing of Tom Hyder, a beloved member of our community who served on our Board of Directors from 2011 to 2016, including a distinguished tenure as President. Tom’s leadership was instrumental in guiding MNHPC with wisdom, compassion, and a steadfast commitment to our members. A mentor to many, Tom's legacy lives on through the many lives he touched and the lasting impact he made in the fields of hospice and gerontology.
