The linked article from the AMA include some important of examples of the ways that health systems are reaching patients where they are, in their homes, improving care, improving access to care, addressing patients needs. Medically Home recognized from its beginnings that bringing acute care to the home, by nature changes the dynamic between a provider and a patient where they are working together, aided by insights that come from being with the patient in their own environment. Medication reconciliation is another example, but there are so many and they are powerful, reflecting that hospital at home care is changing the delivery of healthcare. After decades unfulfilled projections for change, Medically Home's role driving the transformation offers an amazing vantage point. https://lnkd.in/gXSusBT8 #hospitalathome #healthcareinnovation

Great to see more health systems and hospitals adding care for unhoused patients to their home hospital repertoire. Many Medically Home partners have worked within their patient communities to serve those experiencing homelessness and improve access to hospital care through the decentralized model. Improving access to hospital care is one of the model's primary goals. https://lnkd.in/gV8nX8ny #hospitalathome

This morning, Medically Home announced it has partnered with SCP Health to enable virtual clinical command centers, a resource that can help resource-constrained hospitals offer hospital at home care to their patients. While many hospitals and health systems have applied for and been granted the waiver through CMS, some are not yet operating home hospitals. In Medically Home's experience, access to expert logistical, and technological support as well as resources like an established, staffed virtual command center, can remove the barriers and allow more patient access to this model of care. The AHA president & CEO, Rick Pollack, posted an article last week laying out some the benefits of hospital at home care and the need for the waiver that enables reimbursement through the Centers for Medicare & Medicaid Services (CMS) to be extended, saying: ...It [hospital at home care] has emerged as an innovative and promising approach to providing high-quality care in a setting that many patients would choose as an option if available.?Hospital-at-home programs get a thumbs up not only from a majority of patients, but from care providers too, according to a recent?report?from the Centers for Medicare & Medicaid Services that found patients and caregivers overall had positive experiences in the program. Both of these innovative approaches to patient care have proven their worth. However, the waivers that have supported their expansion are scheduled to expire at the end of this year unless Congress acts. That is why the AHA has made making these flexibilities permanent or extending them a top advocacy priority when Congress returns for a post-election lame-duck session. Medically Home's which has enabled care for more than half of the patients referenced in the report mentioned above, is that there are clinical advantages to care at home for many patients who may not fare as well in a facility. Making these flexibilities permanent or extending them as the AHA recommends would preserve reimbursement for an important option for patients and the opportunity for dynamic capacity for health systems and hospitals while setting the stage for broader adoption with commercial payers. #hospitalathome https://lnkd.in/gn8wxYRi

After enabling care for more than 45,000 patients at home, Medically Home finds that care coordination, biometric monitoring, logistics and fulfillment are the keys to being able to deliver hospital at home care. In fact, a care platform that manages all of those pieces is the technology story that isn't being told. "While remote patient monitoring (RPM) technology often dominates discussions about enabling hospital-at-home care, RPM has now become a commodity because the most significant technological breakthrough lies in automating care coordination and physician order fulfillment..." said Medically Home CEO Graham Barnes. #hospitalathome #decentralizedcare https://lnkd.in/gzz9GwZh
