In 2024, 728 hospitals will be penalized by HACRP, with some losing over $1 million in Medicare reimbursements. Larger health systems can typically absorb these losses, but smaller hospitals—especially those in rural or underserved areas—often can't. These penalties can lead to staff cuts, scaled-back services, or even closures, which worsens health access in areas that already lack resources. A Practical Path Forward Experts argue that hospitals don’t need costly overhauls to improve safety; many issues can be reduced with effective, low-cost measures like hand hygiene and advanced cleaning technologies. This layered approach supports safer outcomes for patients without excessive financial strain. Prioritizing Health Equity To create real improvement, CMS and policymakers should balance quality incentives with the operational realities of under-resourced hospitals. HACRP could benefit from a structure that rewards incremental improvements and supports hospitals serving disadvantaged communities.
