This event is designed for healthcare professionals and community leaders committed to advancing health equity. In recent years, efforts to address health disparities have gained significant momentum. Many of these efforts have centered around raising awareness and educating organizations and individuals on systemic injustices, structural and interpersonal racism, and health disparities. While there is continued work to be done on this front, there is also an imperative to act to not only understand these forces but address them through structured, measurable processes and objectives. This forum will focus on the transition from theory to actionable practice, highlighting successful models and frameworks utilized by various organizations across the healthcare spectrum. Reports are available for the Kentuckiana region, Kentucky, and each of Kentucky's Area Development Districts.
