The Leapfrog Group’s fall 2024 Hospital Safety Grades assigned a grade to over 3,000 U.S. hospitals on their ability to protect patients from medical errors, injuries, accidents, and infections. Join the Kentuckiana Health Collaborative on December 10 for the last Community Health Forum of 2024, "From Theory to Practice: Operationalizing Health Equity Efforts." This event is designed for healthcare professionals and community leaders committed to advancing health equity. In recent years, efforts to address health disparities have gained significant momentum. Many of these efforts have centered around raising awareness and educating organizations and individuals on systemic injustices, structural and interpersonal racism, and health disparities. While there is continued work to be done on this front, there is also an imperative to act to not only understand these forces but address them through structured, measurable processes and objectives. This forum will focus on the transition from theory to actionable practice, highlighting successful models and frameworks utilized by various organizations across the healthcare spectrum.
