Erie County, Catholic Health, Kaleida Health, ECMC, and LiveWell Erie are working on the 2025-2030 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The CHA is a report on the health and well-being of the people who live in the county. The CHIP is a blueprint of plans to achieve specific goals for community health improvement over the next six-year period. It is critical to involve the community in this process to learn about the most important health conditions and challenges to healthy living in Erie County.
