Tracy* was in urgent need of refills of her seizure medications when she came into the JaxCareConnect program in March of 2024. She was immediately helped by one of our Patient Health Advocates and was connected to our primary care clinic partner, Community Health Outreach (CHO).? When she needed a CT scan and a neurologist, Community Health Outreach referred her for specialty care through WeCareJax at no cost. Due to her epilepsy, Tracy was unable to drive and needed dental care for broken teeth that were caused by her seizures. Her Patient Health Advocate connected her with medical transportation and dental care through community resources. Tracy shared that “the case managers [at JaxCareConnect] really handled with care as well as compassion and really take time to listen to a person's true needs. JaxCareConnect is a program of WeCareJax. We were created by the local nonprofit safety net of free and charitable clinics to assist uninsured residents of Duval County in finding a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to care in order to support best possible health outcomes. Navigating health care can be overwhelming. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their healthcare journey. JaxCareConnect aims to better inform uninsured neighbors about their health care options so that all Duval County residents have access to comprehensive primary care — regardless of insurance status or ability to pay.

This week, we’re highlighting Adah Belle Samuels Thoms, 1870- 1943. “Adah Belle Samuels Thoms was a devoted nurse who co-founded the National Association of Colored Graduate Nurses and fought for equal employment opportunities for African Americans in the American Red Cross and U.S. Army Nurse Corps.
