Today, the room was packed with so many amazing healthcare leaders from our city. In our 2 month soft launch alone: 120 virtual visits that prevented an ER trip >300 nursing care navigation consultations 1400 post acute and transitional care touchpoints Estimated >$500,000 in cost savings This is an example of true collaboration in public health, and we are just scratching the surface!

JaxCareConnect will be closed on Thursday, 9/26 and Friday, 9/27 due to the incoming storm. For those that need to evacuate, shelters open at 4pm today. Jacksonville Transportation Authority will provide free transportation. Medically needy shelters (that are pet friendly) are: Atlantic Coast High School and The Legends Center on Soutel.
