Once a patient reaches End Stage Kidney Disease, treatment options are fairly limited and terrible (dialysis or transplant). And most patients wait years for a transplant - 90,000 currently on the list. Of those, 13 will probably die today - even though 30% of donor kidneys will be discarded this year It's tragic, and terrible, and a major CMS focus. We're hopeful that the newly finalized Increasing Organ Transplant Access Model will succeed in expanding access to transplant. #nephrology #kidneytransplant #esrd #renaldisease #eskd https://hubs.li/Q02ZGTyh0

“Cancer patients are trapped in the overlap of extreme medical and financial burden. They need continuous between visit care and support that our health systems all too often either ration or simply cannot afford." --- CEO Nabeel Kaukab. We're grateful for the opportunity to team up to provide better comprehensive care for these most vulnerable patients. We've long been impressed with the quality of care provided by the nurses of Sweeten Health and with Integra Connect's extensive experience in oncology care transformation across the largest oncology provider footprint in the United States. Together, we will help more oncology practices provide consistent, proactive care for their patients between visits while enabling a financially sustainable shift to value-based care. #OncologyNews #ChronicCareManagement #AVBCC24 https://hubs.ly/Q02Tp-730

We all recognize October as Breast Cancer Awareness Month, but breast cancer doesn't impact all of us equally. Women (and men!) who face health inequities are more likely to have poor outcomes. According to the National Breast Cancer Foundation, Inc.: --- 30% of uninsured women were up to date with breast cancer screening in 2018 compared to 64% of insured women --- 49% of uninsured women delayed or went without care due to cost --- Breast cancer deaths are 40% higher among Black women than white women --- Breast cancer is the leading cause of cancer death for Latin women living in the United States The NBCF shares some resources for breast health education and screening - what are some of the best strategies you've seen health systems use to close the equity gaps here? #BreastCancerAwarenessMonth #HealthEquity https://hubs.li/Q02T01Xk0

Helene damaged a factory that makes IV fluids, reports the The Associated Press. It seems like hospitals will get by for now - but for nephrology providers, the bigger concern over the coming months will likely be the impact to patients who require home dialysis and may have a delayed start. #HurricaneHelene #HomeDialysis #Nephrology https://hubs.li/Q02SpqKq0
