#HWCLI Secured Medicaid 1115 Waiver Grant to Strengthen Social Care on Long Island: It is complicated, necessary work to build coordinated systems for our most vulnerable Medicaid neighbors. Work that we love doing at Intrepid Ascent! The Health & Welfare Council of Long Island (#HWCLI), was approved to lead the Health Equity Alliance of Long Island (#HEALI) Social Care Network, accelerating efforts to address social drivers of health or health-related social needs (HRSN) and strengthen community connections across Long Island. In partnership with Intrepid Ascent, a longstanding ally and Medicaid transformation change agent, #HWCLI is poised to advance these critical initiatives over the next several years. It’s an honor to collaborate with a community-based network of providers to build a program that will help the Long Island community. We know this program is tasked with establishing a “new normal” for how care is delivered through partnerships between healthcare and community-based organizations. It’s important to emphasize that this is a systems change project, and we are fully committed to approaching it as such. Our collaborative planning process firmly anchors on the needs of the Long Island community and their network of local community providers. It's been great to partner with Pivotalogic over the last couple of years to support organizations better understand their privacy and security needs. In both #CalAIM and in with the #NYHER1115waiver there is a need to increase capacity of CBOs, Social Care Networks, and hubs to engage in data exchange. One component of this is to ensure compliance with applicable laws and regs, and in some instances to become #HITRUST certified.
