First diagnosed with chronic kidney disease in his early 30s, Kyle Oldfield, MBA, CPM took a proactive approach to his health by learning everything he could about his condition. He built a network of support and found a kidney donor before the need for dialysis arose. Innovative health plans are increasingly investing in value-based kidney care for their members. Why? Chronic kidney disease is highly complex, affects 15% of the U.S. population, and accounts for a disproportionate share of healthcare costs. Yet many of the complications associated with CKD can be prevented with early education, proactive treatment, and a personalized, holistic approach—all tenets of value-based kidney care.
