Welcome to the Integrative Palliative Institute. Our mission is to help clinicians and caregivers reduce stress, overwhelm, and feelings of losing themselves while caring for ill or aging people. We also assist palliative care organizations in differentiating themselves, attracting patients, and optimizing their clinical and administrative teams. We help our clients through online courses, coaching, consulting, speaking engagements, The Integrative Palliative Podcast, and Dr. Chiaramonte’s book, "Coping Courageously: A Heart-Centered Guide for Navigating a Loved One’s Illness Without Losing Yourself." We empower clinicians and caregivers with integrative approaches for symptom reduction, skills for difficult conversations, positive responses to stress, and micro-wellness tools to enhance their own wellbeing. For organizations, we provide strategies to boost referrals, expand patient care options, improve employee wellbeing, and increase team productivity.   • Integrative symptom management is a different approach to integrative medicine. Much of integrative medicine is about prevention of disease - how can you argue with that? BUT if you feel nauseous and exhausted from chemo, have chronic pain that impacts your quality of life, or can't sleep because your brain won't turn off, prevention may not be the primary thing on your mind. There are many ways to add an integrative approach into your practice. ?? You can do a 2 year fellowship: The Univ of Az has a robust, well-respected fellowship. If you want to pursue Board certification in integrative medicine you'll need to do a program like this. ??You can take an academic program: The University of Maryland Graduate School has a 1 year academic program in integrative health. I teach in this program, and it's great. If you want academic credit you'll need this kind of program. ??You can learn from me: Perhaps you aren't looking for Board certification or academic credit. If you just want to learn more evidence-supported, effective tools to help your patients (and yourself), check out the program below. It is cost effective and uber practical (no fluff) so you'll be able to implement new approaches right away. There is an option to learn in community if you don't want to take this journey alone.     My dad felt trapped in his failing body and desperate to find a solution. He wanted Medical Aid in Dying, but didn't have access to it. This week's episode of The Integrative Palliative Podcast is called Medical Aid in Dying: A Tale of Autonomy and Desperation. It is a vulnerable and painful story. It is the story of my dad's desperation when his body was failing but his State didn't have a way to give him autonomy over the last part of his life. His desperation led him to make a decision that caused pain for him and pain for those who love him. If he had had access to Medical Aid in Dying, his last moments on Earth could have been peaceful instead of painful. He could have felt love instead of fear. He wouldn't have had to be alone. As a palliative care physician I have always been in support of access to Medical Aid in Dying, because it simply made sense to me to give people autonomy over the last part of their life. As a daughter of a gentle, creative, and fiercely independent man who ended his life because he felt trapped by his ailing body, I am broken-hearted that so many people in this country are suffering as he suffered. Forcing people who want autonomy over the end of their life to choose between dying slowly or figuring out how to end their life is unkind. Leaving families to sort out self-imposed aid in dying from "suicide" is cruel. From death certificates to life insurance to telling the neighbors what happened, the way that a terminally ill person leaves this Earth matters. Access to Medical Aid in Dying matters.     ???The Most Powerful Gift You Can Give Someone Who Is Grieving When someone you care about is grieving, you probably aren't sure exactly how to help. You want to say the right thing, show up in the best way, and ease their pain at least a little. You know you can't fix it, so what should you do?? What the grieving truly need is for us to?bear witness. We can't fix it, but we can show that we see it. We can bear witness to their pain and to the specialness of the person they have lost. ???Bearing witness to their pain means sitting with them in their sorrow, without rushing to change the subject or offer a silver lining. It means saying, "I know this is hard. I see how much you’re hurting, and I’m here with you." ???Bearing witness to the specialness of their loved one is just as important as bearing witness to their pain. Say their loved one's name. Ask about them, and honor how they impacted this world. You can say, "Tell me about your mom —what was your favorite thing about her? What did she like to do? How would her friends describe her?"? Grief isn’t only about loss; it’s also about love. By actively acknowledging both the loss and the love, we honor both the person who died and the impact of losing them. As a physician I’ve had the honor of walking with many families as they navigated the loss of someone they love. As a daughter, I’ve just lost my beloved dad in a complicated and traumatic way. These past two weeks have been challenging, and I’ve noticed that what helps the most is when people bear witness to either my pain and loss OR when they bear witness to the unique and interesting way that my dad carried himself through the world. It matters how we show up for each other, and bearing witness is a loving act. My dad was an engineer and an artist. He carved the shelf below out of a tree stump and used it to hold art supplies. I love this shelf. I see his engineer self and his artist self within it. Thank you for bearing witness to his uniqueness.     Helping people cope with deeply upsetting situations is what we do in palliative care. Here are some things that can help… ?? Allow the feelings in as you are able. You don’t have to flood yourself with the feels and you also shouldn’t permanently stuff them. All feelings are allowed - sadness, anger, disbelief, grief. ?? Don’t compare your insides to other people’s outsides. Everybody copes in their own way and that’s OK. ?? Use your senses to focus on the now. What do you hear, feel, see? Focus on the taste of your coffee or the softness of your puppy’s fur. Sometimes your “now” will include planning for the future. But don’t indulge your mind’s tendency to catastrophize about the future with “what if…” ?? Make a list of what you are grateful for - try to fill a notebook page. ?? Put good into the world or help someone else. Life is hard. And also beautiful. And also hard. We can’t choose what happens to us, but we can choose how we respond.
