Using AI and Data Science to Reduce Hospital Readmissions InfoWorks had the opportunity to co-present at the World Conference Forum in San Diego on February 20 regarding how AI and data science integrated within a cloud-based data management architecture can help facilities address readmissions. InfoWorks Principal, Aaron Wells, PhD, detailed how Admission, Discharge, and Transfer data combined with medical and pharmacy claims + unstructured clinical notes can be merged into real-time, AI-based prospective algorithms to risk rank discharged patients according to their likelihood for readmission. Further, he explained how these algorithms can provide tailored insights into the patient-specific attributes that signal readmission likelihood and suggest clinical actions the care team can take to help ensure patients do not end up in the hospital again.
