Clinical Overview of the Vagus Nerve (Cranial Nerve X) Anatomy & Function ? Type: Mixed nerve (sensory, motor, and parasympathetic). ? Origin: Arises from the medulla oblongata. ? Pathway: Exits the skull via the jugular foramen, travels through the neck, and innervates structures in the thorax and abdomen. ? Major Functions: ? Sensory: External ear, pharynx, larynx, and thoracoabdominal organs. ? Motor: Muscles of the pharynx, larynx, and soft palate (except stylopharyngeus and tensor veli palatini). ? Parasympathetic: Regulates heart rate, digestion, and respiratory function. Clinical Examination 1. Palate Elevation: ? Ask the patient to say “Ah.” Asymmetry in palate elevation suggests vagus nerve dysfunction. 2. Gag Reflex: ? Stimulus applied to the posterior pharyngeal wall; tests afferent glossopharyngeal and efferent vagus nerves. 3. Voice and Swallowing: ? Check for hoarseness (recurrent laryngeal nerve involvement) and dysphagia. Common Clinical Manifestations 1. Dysfunction of the Vagus Nerve: ? Hoarseness: Due to recurrent laryngeal nerve damage. ? Dysphagia: Impaired swallowing from motor dysfunction. ? Loss of Gag Reflex: Indicates afferent or efferent pathway disruption. 2. Vasovagal Syncope: ? Overactivation leads to bradycardia and hypotension, causing fainting. 3. Gastroparesis: ? Delayed gastric emptying due to vagus nerve impairment. Causes of Vagus Nerve Injury 1. Trauma or Surgery: ? Thyroidectomy or carotid endarterectomy may damage branches like the recurrent laryngeal nerve. 2. Compression: ? Tumors, aneurysms, or base of skull lesions. 3. Neuropathies: ? Diabetes, viral infections, or autoimmune diseases. Therapeutic Relevance 1. Vagus Nerve Stimulation (VNS): ? Used for epilepsy and refractory depression. 2. Management of Gastroparesis: ?
