The aftermath was heart-wrenching: lives lost, families devastated, and a company left grappling with questions of accountability. An investigation is still ongoing, but it looks like the pad eye was the weak link—a flawed design, poor material selection, and insufficient inspection contributed to the failure. The tragedy wasn’t just an accident; it resulted from neglecting the importance of proper qualifications in safe lifting operations. Certification had given the team the credentials, but qualification—the deeper knowledge of pad eye design, load analysis, and failure prevention—was missing. LinkedIn Expert, Let's talk about Pick and Carry Crane During Tandem Lift. Multiple crane lifts are among the most technically complex to plan and execute, especially tandem lifts involving pick-and-carry activities. We must address some critical points before we perform the job. I will share some of the critical points of this activity: 1. Real-time information and communication for operators to ensure lift and travel are carried out in unison 2. Side loads/forces acting on the crane boom or unintended load distribution 3. Ground preparation and the compounding ground pressures if cranes are close to each other 4. Load sharing for rigid structures requires special attention, as weight transfer is critical when lifting or placing a rigid load. 5. Cranes must travel in the same direction. 6. If pick and carry, lift the load, then move into position. Don’t do both, and make sure all cranes travel in the same direction. Do you have any critical points of this activity that must be added?
