The future of care at home?is evolving, and post-acute providers are ready to handle the moving pieces. In January 2026, the Centers for Medicare & Medicaid Services' (CMS) Transforming Episode Accountability Model (TEAM) will take effect, allowing Medicare to hold over 700 participating hospitals accountable for the cost and quality of care delivered during specific episodes of care, while incentivizing improved patient outcomes and care coordination. Post-acute providers and home health companies play a crucial role in TEAM enablement—providing hospitals and health systems with valuable patient data, advanced technology, workforce support, and more. From insightful conversations on the show floor to engaging discussions during David Zimmerman's panel, this week was packed with inspiring moments and meaningful connections. ? A few key themes stood out this year: AI is shaping the next wave of healthcare transformation, not replacing clinical care but augmenting it to enhance efficiency and decision-making. Providers aren’t wary of advanced technology—they’re wary of bad technology. We need solutions that truly enable better care without adding unnecessary complexity. Great analysis on Acute Hospital Care at Home Program outcomes by Mary Oberst, MS-HEOR and Chi-Cheng Huang. Patients on CMS Waiver at Inbound Health had a 5% lower 30 day readmission rate compared to matched patients based on severity of illness. Also, the mean length of inpatient stay was shorter with the Inbound Health cohort of patients.
