The Institute for Healthcare Improvement (IHI) is a leading, globally recognized not-for-profit health care improvement organization that has been applying evidence-based quality improvement methods to meet current and future health care challenges for more than 30 years.     Productivity. We go through waves of narrative in the NHS - you'll remember 'value for money', 'organisation with a memory', 'one version of the truth', 'duty of candour', and so on. Productivity is part of our current narrative.     Failures to improve care experience and outcomes are almost always traceable to the top. Despite having this essential knowledge, many efforts to improve care falter. IHI President and CEO Dr. Kedar Mate and Vice President Josh Clark, RN, co-authored an article on the need for care operating systems—a seamless, fully integrated, technology-enabled operating model that, by design, generates continuous learning and improvement. In the article, Kedar and Josh outline the six common elements that, collectively, are called "care operating systems": Valuing People, Appreciating Complexity, Integrating All Dimensions of Quality, Deploying the Quality Team Differently, Communicating in Clinical Language, and Establishing and Maintaining True Transparency.     Today we announced that Dr. Kedar Mate, who has been at the helm of IHI since July 2020, will be stepping down as President and CEO at the end of this year. IHI’s current Chief Operating Officer, Melissa Panagides-Busch, will assume the role of Interim CEO beginning December 1. We hope you’ll join us in thanking Kedar for his indispensable service to IHI, its partners, and everyone around the world who shares his passion for better care and better health. Kedar will remain active with IHI through the end of the year, providing ongoing leadership to the organization during this transition, and will give his final keynote address as IHI’s CEO at the IHI Forum in Orlando next month.     Misdiagnosis—including missed, delayed, and wrong diagnoses—leads to an estimated 371,000 deaths in the U.S. each year, according to a 2021 study, and 424,000 permanent disabilities. Only heart disease and cancer cause more deaths. In a recent article published in WebMD, Ken Terry writes about the scope of diagnostic error in the United States and how patient engagement is crucial to a correct diagnosis. “Patients are the one constant in the process of diagnosis,” adds Patricia McGaffigan, Vice President at IHI. “They are the experts on themselves and the engine of safety, because they can shine a light on things that may not be visible to health care workers.” Read the full article here: https://go.ihi.org/3YN7Q3y
