Is the rapid growth of Medicare Advantage a solution for health care efficiency, or is it a costly gamble that benefits insurers more than patients? On the latest episode of Turn on the Lights Podcast, Don and Kedar are joined by Hayden Rooke-Ley, legal fellow at the Brown University School of Public Health and a senior fellow at the American Economic Liberties Project, to explore the growing impact of private sector involvement, particularly through Medicare Advantage in the U.S. health care system. Medicare Advantage, a privatized version of Medicare, is expanding rapidly, with insurance companies consolidating vertically by acquiring health care providers and facilities. This consolidation raises concerns about cost efficiency and patient care quality, as studies suggest that MA may be costing the government more without clear benefits to patients. Tune in to discover what it means for health care costs, patient care, and the future of Medicare: https://go.ihi.org/3Vaj6po Turn on the Lights Podcast is also available on Apple, Spotify, or wherever you get your podcasts.

“We need to make technology work harder so that we can work smarter in health care.” Ahead of his keynote address at IHI Forum 2024, IHI President and CEO Dr. Kedar Mate discussed the concept of social capital and the need to improve systems—rather than people—in a blog published to the IHI webpage. Kedar describes the concept of having two jobs in health care—the work we are assigned to do, and the job of improving that work. "That approach has been adding effort, not reducing it," he adds, "which seems nearly impossible in today’s stretched clinical care environments." Kedar will be joined onstage at IHI Forum in December by Thomas Lee, MD, Internist and Chief Medical Officer at Press Ganey, for a conversation on building social capital in health care, the growing need for technology-enabled clinical workflows, and an approach to advancing quality that focuses on fixing the systems, not fixing the people. Productivity. We go through waves of narrative in the NHS - you'll remember 'value for money', 'organisation with a memory', 'one version of the truth', 'duty of candour', and so on. Productivity is part of our current narrative. In this episode (I missed it!) with my podcast partner Amar Shah, our brilliant guests discuss the framing of productivity, concepts such as psychological safety and diagnostic lanes, efficiency and value, high quality v low quality buns (!!), partnering with staff and patients and more. Thanks Thea Stein, Chief Executive at Nuffield Trust, and Louisa Wickham, Medical Director at Moorfields Eye Hospital, London and National Clinical Director for Eye Care at NHS England for brilliant reflections. Episode 2 of "Conversations in Health and Care Improvement" is now available, focused on the topic of Productivity. Failures to improve care experience and outcomes are almost always traceable to the top. Despite having this essential knowledge, many efforts to improve care falter. IHI President and CEO Dr. Kedar Mate and Vice President Josh Clark, RN, co-authored an article on the need for care operating systems—a seamless, fully integrated, technology-enabled operating model that, by design, generates continuous learning and improvement. In the article, Kedar and Josh outline the six common elements that, collectively, are called "care operating systems": Valuing People, Appreciating Complexity, Integrating All Dimensions of Quality, Deploying the Quality Team Differently, Communicating in Clinical Language, and Establishing and Maintaining True Transparency. Download the full article here: https://go.ihi.org/40Xbomw On the final day of IHI Forum 2024, Josh Clark will be leading a session on how the IHI Care Operating System approach can enhance patient outcomes and improve clinician experience and operational efficiency. What happens when we reduce people to mere data points in a world driven by economic values? On the latest episode of Turn on the Lights Podcast, Don and Kedar are joined by Marshall Ganz, the Rita T. Hauser Senior Lecturer in Leadership, Organizing, and Civil Society at the Harvard Kennedy School, to discuss how health care and other systems risk dehumanization by reducing people to data points, driven by economic values rather than human complexity. Reflecting on his experiences with Cesar Chavez, Marshall underscores the need for moral, accountable institutions and emphasizes the importance of continuous learning and practical engagement in driving change. Tune in as Marshall Ganz shares powerful insights on transforming communities, challenging the status quo, and reclaiming humanity in systems like health care: https://go.ihi.org/3OfY30V Turn on the Lights Podcast is also available on Apple, Spotify, or wherever you get your podcasts.
