"If you're going to have a premium outcome, you need to have a premium cornea to start with." – Doug Katsev M. D. At #CaribbeanEye, Dr. Katsev emphasized the critical role of #dryeye management in achieving optimal surgical outcomes. He highlighted key treatment strategies, including: ?? Punctal plugs – particularly temporary ones to provide rapid relief while allowing time for other therapies to work. ?? Prescription drops – including cyclosporine and lifitegrast (Xiidra) for inflammatory cases. ?? Patient affordability – leveraging coupons, specialty pharmacies, and staff involvement to improve medication access. ?? MIEBO – showing promising refill rates and patient satisfaction in comparison to cyclosporine. Takeaway: Proactive dry eye treatment is essential for better vision and happier post-op patients. Financial Disclosures: Dr. Katsev reports financials disclosures with Bausch + Lomb

?? IC-8 Apthera Lens & YAG Capsulotomy: What You Need to Know! The other day, I asked which #IOL this was, and many of you guessed right—it's the IC-8 Apthera Lens! ?? This small-aperture IOL is designed for patients seeking near, intermediate, and distance vision, typically implanted in the non-dominant #eye. But where have I seen it work best? Post-radial keratotomy (#RK) patients. The lens helps neutralize their irregular #astigmatism, leading to impressive outcomes. ?? YAG Capsulotomy Tip: If a posterior capsule opacifies, be strategic with your laser shots: ? Aim for the periphery, far from the black filter ring. Hitting the ring can release carbon nanoparticles, potentially damaging the lens and affecting vision. ? If the flap hasn’t fallen, you can also target the center to break adhesion. Huge thanks to my amazing friend and surgeon, Arjan Hura, MD for these pearls! ?? Financial Disclosures: None #Cataract# #IOL #RadialKeratotomy #Ophthalmology Dr. Valentina Lozano, M.D. Lozano

Nick Bruns, OD, FAAO at Summit Eye Care of Wisconsin starts every cataract surgery consultation with one key question: What does the patient’s lifestyle demand? Golf, skiing, reading—matching the right IOL begins with their daily needs. But lifestyle is just one piece. #Eyehealth and precise calculations—corneal regularity, macular health, K values, axial length—all play a role in achieving the best refractive outcome. For post-refractive patients, expectations are high, making light-adjustable lenses a game-changer. They allow vision to be fine-tuned after surgery, making choices less hypothetical and more precise. While the perfect #IOL doesn’t exist, today’s options bring patients closer than ever to clear, customized #vision. Financial Disclosures: Dr. Bruns has disclosures with RxSight

AI is reshaping #ophthalmology, enhancing precision, and transforming patient care. In this video, Dr. Radhika Rampat discusses key innovations: ? AI-Powered Diagnostics – Detecting diabetic retinopathy, #AMD, and even early signs of Alzheimer’s from fundus photos. ? Surgical Advancements – AI-driven topography and tomography improve #LASIK and cataract surgery outcomes. ? Robotic-Assisted Surgery – Enhancing precision in complex vitreoretinal procedures. ? Personalized Treatments – Machine learning models optimize #IOL selection and post-surgical recovery. Whether we like it or not AI is here. Lets remember, AI isn’t replacing eye doctors—it’s empowering them with smarter tools for better outcomes! #eyecare #cataractsurgery #AIinEyecare #optometry

At #CaribbeanEye, Dr. Shamik Bafna (Cleveland Eye Clinic) introduced DSLT (Direct Selective Laser Trabeculoplasty)—a revolutionary leap in laser glaucoma treatment! ?? ?? From SLT to DSLT: SLT has been around for 25+ years, but the LiGHT Trial showed it should be a first-line treatment over drops. Now, DSLT takes it to the next level! ?? Ultra-Fast & Automated: 120 laser applications in just 3 seconds—compared to conventional SLT, which takes 3-4 minutes. ?? No Contact, More Comfort: Unlike traditional SLT, DSLT doesn’t require a lens on the eye, making it more comfortable for patients. ?? Ergonomic & Efficient: Physicians benefit too—just position the patient, let the eye tracker and limbus tracker do the work, and deliver precise treatment in seconds. ?? Accessible & Insurance-Covered: Like SLT, DSLT is typically covered by insurance, minimizing out-of-pocket costs for patients. Is DSLT the future of glaucoma care? Drop your thoughts below! ?? #DSLT #Glaucoma #SLT #Ophthalmology #Innovation #EyeCare Financial Disclosures: Dr. Bafna is a consultant for Alcon References: Wiley W, Bafna S. Early DSLT experience. Dr. Sumitra Khandelwal takes a deep dive into the landmark CLEK study, which shaped our understanding of keratoconus progression. ???? This pivotal trial followed 1,000 keratoconus patients over nearly a decade, confirming key insights: ? Progression is common, especially in younger patients ? Atopic disease accelerates progression ? Visual quality declines over time, even with seemingly good acuity Before CLEK, the impact of corneal irregularity on vision was poorly understood. Now, we know the importance of early screening and treatment, including cross-linking and specialty contact lenses. #Keratoconus #CLEKStudy #Ophthalmology #Eyecare #Eyehealth References: Wagner H, Barr JT, Zadnik K. ?? AI in IOL Calculations: The Next Big Step In Part 2 of our chat with Dr. Radhika Rampat and Dr. Guillaume Debellemanière, we get real about the biggest challenge in using AI-powered #IOL formulas—integration. Right now, most biometers don’t support these modern formulas, meaning surgeons have to manually enter data into online calculators. Not exactly seamless. ?? But there’s good news—teams are working hard to change that. Imagine a future where AI-driven IOL formulas are built right into our biometers, making precision calculations effortless. It’s coming… but how soon? ?? #AI #IOLCalculations #Ophthalmology
