Hudson Headwaters Health Network announces the opening of PACE (Program of All-Inclusive Care for the Elderly) at Hudson Headwaters in Glens Falls. PACE is a community-based program that provides and pays for coordinated medical and social services to qualifying individuals ages 55 and older who may otherwise be nursing home eligible. PACE at Hudson Headwaters is the 10th PACE program in New York State and the first in the state sponsored by a Federally Qualified Health Center.
