The disappointing health status of our state and nation is due in part to the relatively low emphasis of #PublicHealth services compared with “sick care” in the US. As we know, patients at risk have comprehensive problems, so comprehensive solutions are required to be successful. Hippo was designed to convene needed medical and social services in any patient/client encounter. We virtually gather those necessary to meet all needs at the point of contact. The “Last Mile” is the point of frequent failure and why top down programs can be disappointing. That “Last Mile” typically is the product of the Socioeconomic Determinants of Health. As the SHIP program revealed, we must go to the patient’s home and places of the patient’s preference/convenience (such as churches, schools, libraries, local clinics and hospitals) to overcome barriers in order to meet their needs. We overcome the SEDs by leapfrogging them: ?? Bring in family members, influencers, clinicians combined with needed social services, and spiritual support. ?? Avoid the fragmentation of services by collaboration with the patient, family and care givers at the point of care. ?? Provide unity for the the patient at home or places of convenience. ?? Unite Public Health services with unique in-depth remote medical care that only Hippo provides-comprehensive medical care at the point of care most convenient for the patient. Adherence, compliance, and lifestyle change must occur if we are to generate the behavior changes necessary for individual health and the desired health outcomes for our population. The term “Coactive Care” refers to a collaborative care model involving the active participation of multiple healthcare providers with patients. Rural hospitals are the life blood of rural counties. The closure of a rural hospital can significantly harm a local economy by?causing job losses, reducing local spending, decreasing property values, and deterring new businesses from settling in the community, ultimately leading to a decline in per capita income and potentially a population decrease as residents seek healthcare elsewhere – essentially acting as a major economic setback for rural areas that heavily rely on the hospital as a major employer and service provider. Now, health systems can make good on their “hub and spoke” promise by transforming their rural hospitals into “Centers of Confidence,” where the local population has trust that they will receive the same high quality of care as at the regional hospital. This requires a commitment to develop a telemedicine connection to remote specialty physicians from their main campus. By providing remote consultation and guidance by the same specialists that they travel to see at the regional hospital, local hospitals can improve patient access, cost, service and satisfaction. Specialty physicians can guide treatment in rural outpatient clinics, emergency rooms, operating rooms, ICU beds, nursing homes and even home health by remotely supporting primary physicians, ER physicians, hospitalists, general surgeons, nurse practitioners, medical assistants and social workers. According to a?report?by Trillian Health, telehealth volume peaked with 76.6 million visits — or?more than a quarter of all outpatient visits in the US?— in the second quarter of 2020. But while telehealth volume remains far higher than it was before the pandemic, visits have dropped by nearly half since the peak. While telehealth visits for behavioral health?have remained strong?since the pandemic — often, it should be noted, because telehealth is the only option for many patients seeking mental health services — telehealth visits outside of behavioral health have not kept pace.
