This is a culmination of the leadership you'll see here at HRG and spending a decade post USN Nuclear Navy with industry - building a scalable model for becoming a HRO.
I recently came across the Talking About Organizations Podcast series. Their episode on High Reliability Organizations is a good listen. In the podcast they are joined by Tom Mercer, the former Commanding Officer of the USS Carl Vinson. This was the aircraft carrier that Weick and Roberts used in their original study of High Reliability Organizations (HROs) while Mercer was in command. I've long found the five HRO principles very interesting alongside the concept of 'Collective Mind', but struggled with the lack of detail and how to apply the ideas, particularly outside of the military context. So, I particularly enjoyed hearing first-hand how these concepts applied in their original practice. The podcast provides some nice examples of how 'Preoccupation with Failure', 'Sensitivity to Operations' and 'Deference to Expertise' play out in everyday practice aboard an aircraft carrier. Today, the U.S. Chemical Safety and Hazard Investigation Board (CSB) released its?final investigation report?into the November 19, 2023, fire at the Marathon Martinez Renewables facility in Martinez, CA.?The incident seriously injured one employee, who suffered third-degree burns over most of his face and body, and resulted in approximately $350 million in property damage to the facility.?
