At Heartland Country Village in Black Earth, Wisconsin, 20 miles outside of Madison, we know every person has unique needs when it comes to their care, and every person’s path to healing and living a fulfilling life is different. Heartland Country Village is a Skilled Nursing Facility and offers semi-private rooms for short-term and long-term care needs. The 24-hour long and shortterm nursing care is delivered by highly trained, compassionate caregivers who focus on quality of life. Patient care encompasses clinical, spiritual, emotional and social needs, so our dedicated activities department ensures every resident lives and enjoys an active and healthy schedule of available activities daily. Heartland Country Village serves patient and resident needs in each stage of life, as we have for nearly 30 years!
