Last month the NYT wrote an article about telehealth widening mental healthcare disparities rather than closing them. As someone obsessed with #accesstocare, I find this article's framing unhelpful. 1?? First, let's consider that access issues have always existed across demographic categories. Unless you're wealthy (and even then), you've probably encountered issues with looking for mental healthcare. One of the studies cited found that higher income youth used video visits more often than lower income. Higher income was defined by 400% or more of the FPL, so about $107k for a family of three. Not exactly rich. Improved access for higher-income families, urban residents, and white children is still a win. 2?? Second, this article mixes in stats from a few different studies. Some of the studies show differential use of telehealth, and other studies show differential use of mental health services broadly. It's a leap to say increased availability of telehealth is contributing to racial and economic disparities in service use. I'm sure it contributes, but we don't know how much. 3?? Third, I'm not sure why anyone would've expected a single solution (telehealth) to address access barriers for all populations! Black children in rural areas are likely facing different barriers to care than white children in major cities. And both of these groups are likely facing different barriers than adults anywhere. Telehealth has its place and I think will continue to serve some patient populations and therapists well. But let's not pretend that was ever going to fix access to care. Tailored solutions will win out in the end. Let's make sure to boost and promote those. Healwise is working on one such solution, by making it easier to discover affordable options, inclusive of nontraditional supports like support groups and helplines.
