?? IMPORTANT INFO FOR OUR SCN STAKEHOLDERS ?? You may have learned by now that CMS officially rescinded its November 2023 and December 2024 health-related social need (HRSN) guidance on March 4. Since the rescission (linked below), we’ve heard directly from the State and our national partners that the NYHER waiver is unaffected, meaning that the SCN & HRSN Program remains as is. CMS will still review HRSN service proposals on a case-by-case basis. Through our collaborative efforts, we’re confident we can prove our work is a valuable investment! Community transitional services ?? Tenancy sustaining services ?? Housing transition services ?? #ICYMI Last Friday, January 24, New York State Department of Health announced the state's Health Equity Regional Organization (HERO) —?United Hospital Fund of New York?(UHF)! UHF will coordinate HERO strategies with the goal to functionally bridge public health, social services, and health care delivery. UHF will collaborate with the New York State Medicaid Program, health care providers, community stakeholders, and academic partners to understand effective regionally focused approaches to addressing the social needs of Medicaid beneficiaries, such as food insecurity, housing instability, and lack of transportation.
