Social support and community networks These factors can have a profound impact on a person’s well-being. For example, someone living in a food desert may struggle to find healthy food options, leading to higher risks of chronic diseases. Similarly, a lack of affordable housing can create stress and instability, which negatively affects mental and physical health. Healthlincs and its partners offer care management for special needs children and adults with chronic illnesses, often facing gaps in insurance coverage and support for social determinants of health. To address these needs, the Healthlincs Care Resource Fund, established in 2020, provides financial assistance for services and products not covered by insurance, aiming to reduce health disparities beyond promoting healthy choices.
