HealthLinc is a nonprofit organization dedicated to improving health care for the residents of northern Indiana. We hope to enrich our communities’ lives by providing affordable, quality health care for all. HeathLinc is dedicated to providing quality medical, dental and behavioral health services without regards for a patient’s ability to pay. HealthLinc accepts Medicaid, Medicare and Private Insurance and for the uninsured, provides services on a sliding fee basis based on income and serves the uninsured and underinsured in Northern Indiana. We strive to take a holistic approach to health and wellbeing and pride ourselves on creating a “health care home” for our patients. We now have 12 clinics to better serve our community. To date, HealthLinc has provided 25,571 rides for our patients to get to and from their healthcare services, all thanks to Local Initiatives Support Corporation (LISC) and their partnership with Uber. ?? Without this partnership, patients who have limited or no access to transportation would not have been able to get the health care they need, both at HealthLinc and with other specialty providers. Thank you, LISC, for all you do to help us fulfill our mission to create healthy communities! ??
“I’m not sure what would have happened to my patient if LISC’s program didn’t exist,” says Dr. Michael Kozak, a family medicine physician with HealthLinc Community Health Center in South Bend, Indiana. Dr. Kozak is referring to LISC’s Health Access Fund, which provides free access to non-emergency medical transportation through a partnership with Uber. The program serves people who lack access to reliable transportation, helping them get to and?from the destinations that contribute to their health and wellbeing. To date, 23 organizations – including Dr. Kozak’s – have booked close to 320,000 Uber rides for people in 88 counties across 23 states. In 2023, Erica was selected as a HealthLinc Hero due to her efforts in the face of a patient emergency. Erica quickly jumped into action when she received a call from a patient experiencing distress. Instead of giving up on the situation when the patient hung up in anger, Erica contacted local authorities and worked with them to locate the patient and get them to the hospital where they could receive the care they need.
