JOB OPENING: Social Worker (MSW) - PRN (on-call) status, Harbor Hospice Will Assist Social Work Department up to 1x/week and also with weekend rotation ? Harbor Hospice is a community-based non-profit hospice serving a five-county area along the lakeshore of West Michigan. Since our founding in 1982 our mission has been to provide high quality end of life care, serving all with ’empathy, expertise and excellence’. ? Comprehensive orientation, training and resources provided. QUALIFICATIONS Is a graduate of a Master’s program in social work accredited by the Council on Social Work Education and holds current certification in the State of Michigan or has a Master’s degree in psychology, sociology, counseling, human services or other field related to Social Work who would be supervised by an LMSW. Current State of Michigan Social Work License (LMSW or LLMSW) and Drivers License. SUMMARY OF JOB RESPONSIBILITIES Coordinates social work services for the home care program or Poppen residence in a facility setting. Provides supportive counseling, anticipatory guidance and assistance to patients and their families with personal, psychosocial, psychoeducation, financial and environmental difficulties related to the patient’s illness for patients at hospice residence or in the hospital.
